A Special Caring Environment
The Transitional Care Unit (TCU) at Harford Memorial Hospital offers a special caring environment for patients who no longer require all the resources of an acute hospital but who are still too sick or technology-dependent to be cased for at home. The 17-bed unit is the first choice for care immediately after an acute care hospital stay because it offers:
- A team of dedicated health professionals,
- Warm, home-like atmosphere, and
- Quality of care our patients have come to expect from Upper Chesapeake Health.
The TCU's primary area of services include:
- Orthopaedic Rehabilitation
- Infusion Therapy
- Cardiac Recovery
- Reconditioning and Strengthening
- Post Surgical Recovery
- Oncology and Pain Management
- Neurological Management
- Complex Medical Management
- Skin and Wound Care.
Patients benefit from individualized care plans developed by an interdisciplinary team of medical and therapeutic experts. Led by a physician, the team includes:

- Registered Nurses
- Physical and Occupational Therapists
- Speech Language Pathologists
- Registered Dietitian
- Care Manager
- Activities Coordinator

Healing Environment
Most patients are admitted to the Transitional Care Unit from an acute unit of a hospital for continued care at a less intensive level. The healing environment on the TCU helps ease the patientÕs transition to home or an alternative care setting. For the comfort and convenience of our patients. the TCU offers:
- Residential-like private and semi-private rooms that are ADA compliant
- Complimentary local telephone and cable television service
- A dedicated dining area for residents.
- A comfortable and attractive day room where residents can relax and enjoy hobbies and recreational activities.
- A dedicated rehabilitation center within the Unit offering a full range of therapeutic services seven days per week.
- Access to all the resources at Harford Memorial Hospital including our on-site pharmacy, IV therapy, imaging services and laboratory.
