Welcome to Anticoagulation Services at Upper Chesapeake Health. Upper Chesapeake Health is committed to providing the highest quality care for all patients. Description of Service
The Anticoagulation Service Center has been established to provide high quality care to patients who have been prescribed oral anticoagulation medications such as Coumadin (warfarin).
This service is managed by pharmacists with special training in anticoagulation management. Our dedicated team will closely monitor the progress of your anticoagulation therapy through PT/INR lab tests obtained by a finger-stick.
The results will be used to adjust the dosage of your anticoagulation medication for optimum therapy and safety.
Our patients will receive education about their medication, their prescriptions will be called into the pharmacy of their choice, and follow-up appointments will be scheduled.
Referral and Pre-Registration
If your physician prescribes anticoagulation medication for you and feels that you would benefit from our services, he/she will complete an Anticoagulation Referral Form. This form will be submitted to the Anticoagulation Services Center, and you will be contacted by one of our team members to schedule your first appointment.
Before the day of your first appointment, you will be contacted by ScheduleFirst to complete pre-registration by phone. If you need to cancel or change an appointment, please contact the Anticoagulation Services at 443-643-2270 ( Upper Chesapeake Medical Center) or 443-843-5570 ( Harford Memorial Hospital).
First Appointment
You will receive a reminder letter in the mail if your appointment was scheduled at least five days in advance. Please arrive at the Anticoagulation Services Area 15 minutes prior to your first appointment to allow time for registration, and 10 minutes before the start of each successive appointment.
For your first appointment, you will need to bring a photo ID , insurance cards, and a list of your medications to include prescription, over-the-counter medications, and supplements.
Each appointment will include a finger stick for INR/PT results, and one-on-one consultation with one of our specially trained pharmacists.
