Home Care Eligibility

To receive home care services, patients need to:

  • Be homebound - requiring a taxing effort to leave home
  • Require a skilled professional service
  • Be under the care of a Maryland licensed physician

Arranging For Home Care
Home care services are available seven days a week with access to a clinician 24 hours a day. Upper Chesapeake/St. Joseph Home Care offices are open Monday through Friday from 8:30 a.m. to 5 p.m. Referrals can be phoned to our office at: 410-931-0990 or          1-877-640-1809.

Additional Services
Upper Chesapeake/St. Joseph Home Care also offers the following services:

  • Harford Hospice 
      - Providing care to terminally ill patients and their families in Baltimore City, Baltimore, Harford and Cecil counties.
  • Choice Care 
      - Providing companion and personal care services to the Upper Chesapeake/St. Joseph Home Care service area.

Our Multidisciplinary Team

  • Registered Nurses
    • Provide skilled assessments and instruction related to your medical situation.
  • Physical Therapists
    • Assist with muscle strengthening and mobility with a focus on home safety and patient comfort.
  • Occupational Therapists
    • Teach you to accomplish activities of daily living, such as dressing, bathing, and food preparation.
  • Speech Therapists
    • Work with you to improve speech, swallowing and cognitive problems.
  • Medical Social Workers
    • Provides counseling to assist you to adjust your illness as well as providing information concerning resources available through your community, state and federal agencies.
  • Home Health Aides
    • Offer personal care services under the guidance of the registered nurse.
  • Dietitians
    • Assess your nutritional status and assist with individual meal planning.

History of Upper Chesapeake/St. Joseph Home Care

In 1999, Upper Chesapeake/St. Joseph Home Care was formed from the merger of the home care companies of Upper Chesapeake Health and St. Joseph Medical Center. This dynamic combination enabled Upper Chesapeake/St. Joseph Home care to serve a larger population while utilizing the strengths of two major Maryland health care systems. We employ over 140 team members who are dedicated to providing the highest quality of patient care. Our team members exhibit a strong commitment to customer service. Valuing the patients' needs and desires has helped us maintain superior patient satisfaction levels. Our number one priority is meeting the needs of our patients.
