Home Care Eligibility

To receive home care services, patients need to:

• Be homebound - requiring a taxing effort to leave home
• Require a skilled professional service
• Be under the care of a Maryland licensed physician

Arranging For Home Care
Home care services are available seven days a week with access to a clinician 24 hours a day. Upper Chesapeake/St. Joseph Home Care offices are open Monday through Friday from 8:30 a.m. to 5 p.m. Referrals can be phoned to our office at: 410-931-0990 or          1-877-640-1809.

History of Upper Chesapeake/St. Joseph Home Care

In 1999, Upper Chesapeake/St. Joseph Home Care was formed from the merger of the home care companies of Upper Chesapeake Health and St. Joseph Medical Center. This dynamic combination enabled Upper Chesapeake/St. Joseph Home care to serve a larger population while utilizing the strengths of two major Maryland health care systems. We employ over 140 team members who are dedicated to providing the highest quality of patient care. Our team members exhibit a strong commitment to customer service.
