Electronic Health Record Remote Access – For Physicians and Staff

	If you are a Upper Chesapeake Health on staff physician, you can skip steps 1-5.
	Please follow the steps below in order to pursue remote access to Upper Chesapeake's physicians portal.
	Step 1:	Click here to see if your office has the system requirements for the web portal to Meditech V6
	Step 2:	Complete the application for Remote Meditech Web Portal Access.
	Step 3:	Review the HIPAA Education Packet and complete the HIPAA post-test. Download, complete and fax the post-test to 443-843-8916.
	Step 4:	Complete the confidentiality form, download here. Fax form to 443-843-8916.
	Step 5:	Check your email after you submit your request for follow up communication from UCH.
