Primary Care Clinic

The Primary Care Clinic provides primary health care for residents of Harford County and parts of Cecil County who are uninsured and who meet the designated financial income criteria. Upper Chesapeake Health physicians and nurse practitioners provide preventative health care and management of general health problems for participants of the program.

The Clinic also provides:

  • Vaccinations
  • Limited Diagnostic Tests (Laboratory & X-rays)
  • Help with Obtaining Free or Low Cost Medication
  • Individual Health Education & Information
  • Help with Referrals to Specialists

Who is Eligible?
Residents in the service areas of Upper Chesapeake Medical Center and Harford Memorial Hospital ages 19 and above who meet the income requirement and meet one of the following insurance criteria:

  • Uninsured;
  • Have Primary Adult Care (PAC) - If you need an application, click here.
  • Have Medicaid
  • Have Medicare and are unable to meet the 20% co-pay;
  • Low income; and
  • Homeless (Healthcare for the Homeless Project).

Upper Chesapeake Primary Care Clinic follows the Annual Federal Poverty Guidelines at the 300% level. Proof of eligibility required annually.

Interpretive services are available with advance notice.

Clinic Locations
Clinics are held at various locations throughout the county in the HealthLink Medical Mobile Van or at the HealthLink Wellness Center located at 2027 Pulaski Highway, Suite 206, Havre de Grace, MD 21078.
